THE MISSION OF THE FLORIDA DIABETES ALLIANCE IS
TO COORDINATE RESOURCES TO IMPROVE SYSTEMS FOR DIABETES PREVENTION AND CARE THROUGHOUT FLORIDA.

Goal One

Increase the number of people with, or at risk for diabetes, who have access to quality diabetes prevention programs (DPP) and diabetes self‐management and education (DSME) services.

Objectives

1.1 Educate healthcare practitioners about the benefit and availability of diabetes prevention (DPP) and diabetes self‐management education (DSME) programs

As measured by:
a. The number of practitioners educated.
b. The number of practitioners that implement procedures to screen, test, and refer patients with diabetes for these services.

1.2 Assess, compile and disseminate diabetes prevention (DPP) and diabetes self‐ management education (DSME) resources available at the state, regional and local levels.

As measured by:
a. The establishment of diabetes prevention mentoring program (DPP) implementation protocols.
b. A marketing plan to promote the DPP mentoring program.
c. The number of mentoring contracts the Florida Diabetes Alliance enters into with partner organizations.

1.3 Expand Florida Diabetes Alliance mentoring opportunities

As measured by:
a. The establishment of diabetes prevention mentoring program (DPP) implementation protocols.
b. A marketing plan to promote the DPP mentoring program.
c. The number of mentoring contracts the Florida Diabetes Alliance enters into with partner organizations.

1.4 Market and expand the Florida Diabetes Alliance’s diabetes self‐management education (DSME) mentoring program

As measured by:
a. Protocols for expanding the DSME program.
b. A comprehensive marketing plan designed to expand The Alliance’s DSME mentoring program.
c. The number of DSME contracts The Alliance enters into with partner organizations.

1.5 Identify funding sources to increase access to DSME services in underserved and rural areas of Florida

As measured by:
a. The number of funding sources identified, which could include private, government or foundation grants; individual or corporate contributions; program income; In‐kind services and other revenue streams.
b. The number of contributions received.

Goal Two

Identify barriers to diabetes prevention and care, and develop strategies to address them

2.1 Review and synthesize existing data on diabetes in Florida.

2.2 Identify gaps in the existing data.

As measured by:
a. The creation of a report of available Florida diabetes data.
b. The dissemination of the data report.

2.3 Develop strategies designed to overcome the identified barriers

As measured by:
a. A report outlining recommended strategies.
b. Implementation of the recommended

Goal Three

Facilitate communication and foster collaboration among Florida’s diabetes stakeholders.

3.1 Convene an annual stakeholder summit

As measured by:
a. The number of stakeholders participating in the summit.

3.2 Disseminate stakeholder summit proceedings

As measured by:
a. Distribution of abstracts, a summary of key discussions, and action steps identified at summit.

3.3 Increase the Florida Diabetes Alliance membership, ensuring stakeholder diversity.

As measured by:
a. The creation of a board-approved marketing plan.
b. The dissemination implementation of the marketing plan, to include a critical review of the Alliance membership application and process, member costs and benefits, data illuminating current membership make up

3.4 Develop and maintain an active online presence

As measured by:
a. The number of unique Facebook and other social media visits.
b. The number of social media platforms where The Alliance has a presence
c. An active web site updated quarterly.

Goal Four

Demonstrate organizational integrity

4.1 Establish and strengthen Florida Diabetes Alliance standing committees as per the organization’s bylaws

As measured by:
a. Committees are structured and assigned chairpersons. (Complete!)
b. Quarterly(?) committee meetings held.
c. Committee meeting minutes are sent to the full Alliance board for review (at least X amount of time prior to full board meetings (?).

4.2 Conduct an annual evaluation of the organization’s performance, to include:
a. An objective review of programmatic and operational goals and objectives; measures, benchmarks, targets and outcomes, with recommendations for continuous quality improvement.
b. A board self‐evaluation to include performance in areas such as board composition and recruitment, diversity, committee functions, board member engagement and commitment, meeting effectiveness, board/ED relationship, and other areas determined by the board.
c. An evaluation of the CEO or Executive Director, to include performance in areas such as leadership and staff relations, board relations, collaboration and community relations; finance; marketing, and other areas determined by the board.

As measured by:
a. A board-approved report highlighting key information and action steps.

4.3 Develop and disseminate a Florida Diabetes Alliance annual report

As measured by:
a. Completed annual report.
b. Annual report disseminated.

4.4 Obtain insurance for the board of directors

As measured by:
a. A valid Certificate of Insurance.

4.5 Develop a fundraising plan

As measured by:
a. A board-approved fundraising plan.
